Terms and Conditions for Remote Consultations
1. Introduction
These terms and conditions ("Agreement") govern the provision of remote consultations by Dr. Jeremy Campbell, PT, DPT ("Provider" or "Doctor of Physical Therapy") to individuals seeking non-physical therapy services through virtual means. By engaging in remote consultations, you agree to abide by these terms and conditions.
2. Scope of Services
2.1 The services provided by the Provider during remote consultations are not intended to replace in-person physical therapy sessions. These consultations are informational in nature and do not constitute a doctor-patient relationship.
2.2 The Provider will not diagnose or treat any medical condition during remote consultations. Any recommendations or advice given should not be considered a substitute for professional medical care.
3. Client Responsibilities
3.1 Clients must provide accurate and complete information about their health history, current conditions, and any relevant medical information.
3.2 Clients understand that the Provider cannot physically examine them and, as such, certain limitations may exist in the accuracy of assessments and recommendations provided.
4. Confidentiality
4.1 All information shared during remote consultations will be treated with the utmost confidentiality in accordance with applicable privacy laws and regulations.
4.2 Clients are responsible for ensuring the privacy and security of their communication devices and networks during remote consultations.
5. Limitation of Liability
5.1 The Provider is not liable for any direct, indirect, incidental, consequential, or special damages arising out of or in any way connected with the use of remote consultation services.
5.2 The Provider does not guarantee the accuracy, completeness, or suitability of information provided during remote consultations. Clients are encouraged to seek in-person medical evaluation for comprehensive assessments.
6. Payment and Refund Policy
6.1 Fees for remote consultations are outlined on the Provider's website or communicated directly to the client.
6.2 Payments are due prior to the scheduled remote consultation session. No refunds will be issued once the consultation has taken place.
7. Termination
The Provider reserves the right to terminate or refuse services to any client at their discretion.
8. Governing Law
This Agreement shall be governed by and construed in accordance with the laws of Illinois, US, without regard to its conflict of law principles.
9. Modifications
The Provider reserves the right to update or modify these terms and conditions at any time. Clients will be notified of any changes.
